OPERATOR’S LICENSES  (Bartender’s License):  Proof of completion of the Responsible Beverage Course is required upon application for an Operator’s License.  This requirement is waived if the applicant has held a valid Operator’s License within the last two years in the State of Wisconsin.  Apply at City Hall, 50 Tenth Street during regular hours and allow two days for processing.  Contact Fox Valley Technical College at 715-823-8324 for class schedules, fee and registration information.

Fee's for application are Original($50.00), Renewal($40.00), Provisional($8.00) and Temporary($5.00).  A $20.00 non-refundable fee will be charged if the application is rejected for any reason.

Explanation of questions on above operator’s application

It is very important that your operator’s application is filled out correctly and completely.  Any information that is omitted, whether intentionally or not will result in a denial of your license!  Read this very carefully, and if you do not understand a question, someone from the City Hall or Police Department will assist you.  If your license is denied, you will not be allowed to re-apply for 30 days from the date of denial.

1).  Question one asks for any other names used by you now or in the past.  This should include maiden names, aliases, and nicknames.

 2).  Please take the driver’s license number off your license and enter it on this line.  Also include the state of issuance.  If you do not have a valid driver’s license, please indicate it on the line in place of the license number.

 3).  This line is for convicted offenses in the past five years.  That means traffic citations, ordinance citations, warrant arrests for violations, misdemeanor or felony convictions (you are not required to list parking tickets).  These violations can be charges from a municipality, county, state, or federal and should include DNR violations as well.  If you are unsure of a conviction, please indicate the date and time of the offense charged to the best of your knowledge.  This area should also include violations that occurred as a juvenile (under 18), as those violations will show up on the background check.

 4).  This area is for any currents pending charges or court action you are involved in that has not yet been resolved in the courts. 

 Remember, before signing the form in the presence of a notary public; make sure all the information is accurate.  If you have questions, speak to an employee at City Hall or the Police Department before signing.  Your application will be denied if inaccurate!
